If your provider has not submitted a claim to Allegiance prior to using the online Verification of Benefits system
you will not be able to access benefits at this time. To request a Verification of Benefits you can request them to be
faxed to you through our automated phone system by calling the phone number listed on the patients ID card.

Please note that accounts inactive for more than 24 months are deactivated as a security precaution. If your account has
been deactivated, please create a new account; all prior data will be available through this new account.
