The Tier 1 Network includes the NCH facilities, NCH Medical Group providers, and Obstetrics.

• W4U Tier 1 office visits are eligible for $35 primary care and $45 specialty co-pays.
• W4U Choice Tier 1 office visits are eligible for $50 primary care and $60 specialty co-pays.

Under W4U, all facility services (SURGERY, IMAGING, THERAPY) need to be performed at an NCH facility or an authorization will be needed prior to services elsewhere.

NOTE: PROVIDERS WITHOUT THE NCH LOGO ARE NOT PART OF TIER 1 NCH MEDICAL GROUP BUT HAVE PRIVILEGES AT NCH. IF CHOOSING A PROVIDER THAT’S NOT NCH MEDICAL GROUP, TIER 1, BE SURE TO VERIFY THEY ARE COVERED UNDER OUR CIGNA OPEN ACCESS PLUS NETWORK (TIER 2).
